Another difference between older and newer studies is that the type of patients treated with the ketogenic diet has changed over time. When first developed and used, the ketogenic diet was not a treatment of last resort; in contrast, the children in modern studies have already tried and failed a number of anticonvulsant drugs, so may be assumed to have more difficult-to-treat epilepsy. Early and modern studies also differ because the treatment protocol has changed. In older protocols, the diet was initiated with a prolonged fast, designed to lose 5–10% body weight, and heavily restricted the calorie intake. Concerns over child health and growth led to a relaxation of the diet's restrictions. Fluid restriction was once a feature of the diet, but this led to increased risk of constipation and kidney stones, and is no longer considered beneficial.
There is scientific evidence that supports the use of a ketogenic diet to help manage certain neurologic conditions. There are also medical, nutrition, and fitness professionals that recommend the eating program for patients and clients who have other health goals. But just because the program works for some, doesn't mean that it's the best diet for you.
A systematic review in 2016 found and analysed seven randomized controlled trials of ketogenic diet in children and young people with epilepsy. The trials were done among children and young people for whom drugs failed to control their seizures, and only one of the trials compared a group assigned to ketogenic diet with a group not assigned to one. The other trials compared types of diets or ways of introducing them to make them more tolerable. Nearly 40% of the children and young people had half or fewer seizures with the diet compared with the group not assigned to the diet. Only about 10% were still on the diet after a few years. Adverse effects such as hunger and loss of energy in that trial were common, with about 30% experiencing constipation.
So, we have a situation where the body has way too much insulin in the blood—yet the insulin is not able to do its job in the cells, either. The cells are resisting it. As a result, the body keeps making more and more insulin. These hormones are on a constant feedback loop, sending and receiving messages of “Sugar is high—release more insulin. . . . Must lower blood sugar for the body to stay alive.”
Some benefits of going keto are difficult to dispute. Following a high-fat, low-carb diet can be a solid strategy for rapid weight loss and blood-sugar control. The keto diet can also be great for children with tough-to-control epileptic seizures. For decades, people have seen stellar results managing those conditions on a keto diet with the help and guidance of professionals.
Around this time, Bernarr Macfadden, an American exponent of physical culture, popularised the use of fasting to restore health. His disciple, the osteopathic physician Dr. Hugh William Conklin of Battle Creek, Michigan, began to treat his epilepsy patients by recommending fasting. Conklin conjectured that epileptic seizures were caused when a toxin, secreted from the Peyer's patches in the intestines, was discharged into the bloodstream. He recommended a fast lasting 18 to 25 days to allow this toxin to dissipate. Conklin probably treated hundreds of epilepsy patients with his "water diet" and boasted of a 90% cure rate in children, falling to 50% in adults. Later analysis of Conklin's case records showed 20% of his patients achieved freedom from seizures and 50% had some improvement.
In contrast, multivitamins aren’t a good solution as they are synthetic and lack a lot of nutrients like polyphenols, antioxidants and fiber that green powders and whole food sources provide. And the lack of fats and enzymes make the nutrients they do contain very difficult to process properly. Just because you’re putting something in your body doesn’t mean your body can use it.
There are so many tricks, shortcuts, and gimmicks out there on achieving optimal ketosis – I’d suggest you don’t bother with any of that. Optimal ketosis can be accomplished through dietary nutrition alone (aka just eating food). You shouldn’t need a magic pill to do it. Just stay strict, remain vigilant, and be focused on recording what you eat (to make sure your carb and protein intake are correct).
